What happened
[REDACTED] was making an Oxygen delivery at a St. Als Urgent care in [REDACTED] on [REDACTED]. When he opened gate to access tank for delivery he felt a pop in his left shoulder neck area. Initially he tried to self-remedy with ice and heat but pain continued. He called me and i advised him to contact Medcore and he did. He was advised to get checked out. Reported by driver to [REDACTED] 10 2. Entered 10 6 into Velocity.
