Employee 's right leg and arm were sprayed with 167F 2% caustic due to an improper lock out tag out on the line. The employee fell on their left shoulder in response to the incident.

Employee received 1st and 2nd degree burns on right leg and right arm. They also received a tear in their left labrum.
