Employee was working as a team of three to flow glass through the orifice ring. Glass on the edge had hardened and was being removed. Employees were using T Bar chisels to complete task.

Employee was working as a team of three to flow glass through the orifice ring. Glass on the edge had hardened and was being removed. A large chunk of glass came dislodged and missed the disposal chute. When the glass missed the chute it struck the employee in the right shin causing a 4 laceration.

Four Inch laceration to the right shin above the ankle.

Slightly larger than softball size (described by employee) of hardened glass.
