The employee was completing manual swabbing procedures of the blank side of the IS forming machine. This procedure involves stopping the desired section (of 12) and inserting a lubricated cotton swab to apply lubricant to the mold cavity and surrounding parts.

Based on video review the employee appears to depress the stop button on at least two sections of the machine. Proper procedure would only allow for one section of the machine to be stopped at any given time for swabbing. The employee then turns away from the machine to replenish the lubricant on his swab. When the employee returns to the machine [REDACTED] he mistakenly attempts to insert the swab in a section just to the left of the sections he’d previously stopped resulting in a gob of molten glass falling onto his right hand. The employee was wearing proper hand protection at the time of the incident.
