Before the incident
Walking in to work for the day. Employee was walking down the middle parking garage stairs from Level 2 to Level 1.
What happened
Walking down the stairs from the 2nd floor slipped or tripped. We think possibly rolled down more stairs. Again unwitnessed. - Report from house supervisor: After review of security footage it appears that employee was slowly and carefully walking down the stairs. She appeared to not be feeling well and sat down on the stairs about halfway down. She then leaned back on the stairs. Other employees immediately came to assist and ERT was called. Employee eventually stood on her own and got in a wheelchair to be taken to the [REDACTED]. After getting security to rewind the security footage further you can see employee 's feet coming down towards the stair landing where it appears she fell. It took the employee about 1 minute to get up. She then attempted to walk down the next set of stairs which is where the original security footage review began. She then proceeded to sit on the stairs facing the security camera.

The oxygen flowmeter cause a bump on the fore head. That cause a terrible headache.
