What happened
A disconnect for a welding outlet approximately 15 feet away from the employee had gotten wet during the sanitation process. The disconnect was left on after the last use this resulted in an arc flash. The arc flash blew the face of the disconnect off and created a loud noise similar to a gunshot. Employee reported nasal bleeding and a headache her next working shift.
