The EE stated she entered the room to retrieve blood from the patient when she slipped and fell on the wet floor. She hit her R arm R leg and back of her head.

Floor was shiny and wet from recent moping d t patient fecal matter. No wet floor sign was present lights were off per the EE and door was closed. The patient was neither able to flex to extend abduct and adduct her R leg. She complained of extreme pain rating of 10 10 in R leg and R arm and back of head. She winced and pointed to the area between occipital and parietal lobe. The employee states she entered the room to perform a blood draw and slipped and fell to the floor.
