was watching the patient on a 1[REDACTED]1. Patient was on 1:1 for alcohol withdrawal

On [REDACTED] I Nursing Assistant was working my scheduled shift at Aspirus-St. Luke 's Hospital [REDACTED]. At approximately [REDACTED] I was on a 1:1 with a patient in room 707 of the Medical Unit of Aspirus-St. Luke 's Hospital. This patient was on a 1:1 due to alcohol withdrawal. Through my training and experience I recognized that the patient was experiencing tremors hallucinations and was generally confused. Due to these factors it was unsafe for the patient to be standing in the room due to being a fall risk. The patient was attempting to get out of bed and was not following directions. The patient then became combative and used their closed right fist striking the left side of my head twice. I then utilized a dynamic application of an untrained technique by placing my body over the patient to prevent them from further striking me while I called out for help. Other nursing staff then arrived. Nurse attempted to help place the patient in a hold to prevent further injury to staff

was struck in the head by the patient twice with the patient 's closed fist was kicked by the patient multiple times by the patient and received scratches from the patient on the back of my neck

the patient 's closed fist the patient 's legs and the patient 's foot

Punched in head twice scratches on neck and was kicked. Concussion.
