What happened

Patient was discharged from RBMC on [REDACTED] to a friends home. I was in patient home for hospice admit on [REDACTED]. Patient was minimally responsive unable to get out of bed had been incontinent of bowel and bladder and was significantly soiled. He had been discharged prior to appropriate arrangement of home caregiver plan and his friends were unprepared and unable to assist with needed care. He was on a mattress elevated about 2 feet off the floor by cinder blocks and on a plywood platform I was unable to bring patient to safe handling level. proper body mechanics used to lower my center of gravity as much as possible offer cares of foley placement modified bed bath linen change and patient repositioning. Patient friend [REDACTED] assisted this nurse is using draw pad to move patient up in bed. after bed care of patient my back started hurting with muscle cramps.
