Worker was supervising children on the playground and brought them inside.

She then returned outside because she left her lights on in her car and went to turn them off. On her way back in she slipped on ice on the sidewalk between the parking lot and the main entrance landing on her left side and sustained injuries to her left back left shoulder and left hip. She had throbbing pain but was able to still work.

Bruised left shoulder and pinched nerve sciatica.

Icy sidewalk.
