On [REDACTED] [REDACTED] was removing a shaft from a gear box. He was using a hammer to do this. At approximately [REDACTED] in the maintenance shop the bolt slip and the hammer hit his arm.

[REDACTED] (3rd Shift Maintenance Tech) was using a large hammer type device to remove a stuck rod from a shaft on [REDACTED]. Employee hit the part several times with the hammer in his right hand while holding the part that was already in a vice with his left hand. After several unsuccessful attempts employee missed the part he was attempting to hammer and hit his left hand instead. Video surveillence confirms event. [REDACTED] left his shift early on [REDACTED] WITHOUT reporting the injury to his supervisor. On [REDACTED] [REDACTED] called Office Manager stating he broke a bone in his hand while working on his shop. At first it seemed as if this was a non-work related injury but video does confirm it happened onsite. [REDACTED] took it upon himself to go to the ER on [REDACTED] in the evening where the broken hand bone diagnosis was received. Witness statements can be found in the attachment of this report; video has been saved by Columbia HR.
