Sanitation employee was inspecting forming tubes for flour mega line he was on top of a step stool he pull the forming tubes out of the frame for inspection purposes from the bottom he pulled to much to the point that the forming tubes came out of the frame the tube frame fell off and it went against him he did fell off of the step stool and the forming tubes hit him on his hard hat and his back.
