What happened
Employee was helping with a change over at the press for M3 he went on top of a step ladder (3 step ladder) his right foot got caught entangled in an air hose located in the area one of the corners of the step ladder was very close to the hose and the employee was right at the edge othe ladder. When he moved his foot it was already entangled in the hose causing him to lose balance.
