Before the incident
Employee was carrying a guide tower on a forklift to the staging area
What happened
While staging the Guide Tower the employee made an adjustment that included moving the lift forward. While moving the lift forward the front wheel rolled over the end of a 4-inch diameter pipe causing it to raise up violently toward the driver’s cab. The driver placed his left hand in the path of the pipe to protect his face. The pipe struck the left hand and the left side of the operator’s chest; this impact resulted in two fractured bones in the impacted hand as well as a light bruise to the chest area.
