What happened
Failed to use proper lifting technique when lifting. Due to heavy weight employee suffered strain to lower back muscles. Also it was unnecesary to lift the greaser machine onto the platform. The hose was long enough. Refused medical care after incident. [REDACTED] requested visit to clinic when over-the-counter meds did not help with pain management.
