Employee had completed the change-out of the mold in an injection molding machine which included positioning the mold that had been removed onto the floor using an overhead crane.

When the employee disconnected the crane hook from the eye bold on the mold the eye bolt which is large and heavy rotated from a vertical to horizontal position pinching his right thumb between the eye bolt and the top of the mold.
