Another nurse had accessed the patient 's port and felt like the needle was secure in the septum but she could not get blood return. She had me assess the port and needle. It seemed the huber was up against the septum of the port. I was planning to withdraw the needle slightly so I could pull away from the septum without removing the needle from the patient and having to stick her again. I grasped the port with my left hand and the huber needle with my right hand. The chair was not locked and moved as I was pulling the needle up. This caused me to pull the needle up too far and out of the patient 's skin. The needle punctured through the glove and scraped scratched my skin.
