Ducking underneath a conveyor.
What happened
Employee was moving a pile of preforms over in a gaylord as they began to overfill. Employee was on the opposite side of the preform conveyor from where the gaylord sit. Employee went beneath the conveyor to access the gaylord rather than going around. When he was done he turned and raised his head up and struck a support bar bracket for the conveyor with his forehead.
