Changing out a ladle

[REDACTED] was changing out a ladle he had climbed on top of the ladle to put the pins in it and after it was completed he went to climb down and his foot got stuck and fell to the ground. He fell about 5ft. He is limping cause his ankle is hurting. Went to take him to get a drug test he passed. He said that he did not want to go and get seen. [REDACTED] and [REDACTED] and I tried to get him to go in and he still declined. After he had left we got ahold of him and got him to come back so a supervisor could him into the emergency room. When he was on the phone he told us that his back is hurting.
