Before the incident
I was assessing the patient and all of his IV sites at the bedside.
What happened
When assessing the patient 's IO site the provider determined that they wanted it removed. I went to remove the IO and found that it was quite deep in the bone. I placed my left hand on the patient 's leg for traction as I used my right hand to pull out the IO. As I pulled it out the needle of the IO sliced open my forearm.
