On [REDACTED] at approximately [REDACTED] [REDACTED] [REDACTED] [REDACTED] approached the timekeeper 's office to report an injury for MS. [REDACTED] on the 2nd floor corporate office coffee area. Upon arrival Ms. [REDACTED] was sitting down with an ice pack on her elbow. Ms. [REDACTED] stated that she had just finished mopping the bathroom floor and walk over to the mid-way coffee area when she slipped and fell on her right side striking her right knee her right elbow & the right side of her face with the lower cabinets causing pain. Ms. [REDACTED] refused any medical assistance and stated that she will go home.
