Before the incident
Assisting primary RN in attempting to safely and calmly convince [REDACTED] who is on a legal hold to return to his room. [REDACTED] 's demeanor changed rapidly and he became combative aggressive violent and uncooperative following his sudden attempt to elope which was stopped by ER security.
What happened
[REDACTED] was kicking legs feet and swinging both arms attempting to strike whomever he could whenever he managed to break free. While I kneeling down attaching the restraints to the bed twice [REDACTED] jerked his arm the first time and his leg the second time. When he jerked his arm while I was holding tension on the restraint strap the force of his motion yanked my right arm towards him pulling on my right shoulder and neck. He did the same thing to my left shoulder while we were readjusting the ankle strap. While I was standing at the foot of the bed I heard a noise that sounded like he 'd broken a strap on the ankle restraints so I quickly grabbed the lower straps that connect to the bed so that he would be unable to kick anyone in the head. While I had ahold of that strap [REDACTED] was erratically and forcefully kicking his legs. On his downward kick he slammed his heel into the back side of my left forearm with significant force.

My knee was crushed into the wall. They could not identifty the injury in the x-ray.
