Corner Stone Parking Contingent Worker Valet Parking Attendant sustained trip and fall on Allina Health Buffalo Hospital Property with several injuries fracture to R wrist and R hand contusion R arm and R elbow facial contusion with minor head injury facial contusion. The EE was walking from Day Surgery Entrance to [REDACTED] to retrieve the patients car. The EE looked down at Key Chain tag to verify the correct license plate number of vehicle and tripped over own feet falling forward. The EE outstretched R arm to catch self falling into the car hitting face first then the R hand R arm and R shoulder bouncing off the car and stumbled to the ground. The [REDACTED] glasses shattered and flew off their face. The EE then stood up feeling a bit dizzy recovered then drove the car to the [REDACTED] entrance where EE was met by ER personnel. A patient who witnessed the fall reported it.
