Before the incident
Effected Employee was operating the base tube machine when he then stopped took off his gloves and started to head to lunch break. As he proceeded to the breakroom his line leader asked him to stack steel pallets behind the pallet welder for stock

After lifting up a pallet his hand was cut by the center bridge of the pallet. He did not put his gloves back on for this operation.
