The banding for the pallet kept breaking not adhering together. Employee put machine into hand manual position to do a routine adjustment with the guard in place. Employee was trying to reset the timing with a tool.

While the employee was turning the timing wrench the equipment randomly moved backwards. When the machine moved it pinned employee 's hand between post and wrench.

Employee 's hand was pierced from the wrench resulting in stitches.
