On [REDACTED] an associate was working on 8 mill when they experienced rubber sticking to the back roll The associate grabbed the safety bar with their left hand stepped under it and started to cut the rubber with their right hand The associate maintained a grip on the safety bar while doing this process As the associate was cutting a football sized piece of rubber fell off the mill The piece of rubber struck the backside of the associates head The associate then hit their tooth on the mill knife bar resulting in a chipped tooth
