What happened
Employee changed a gasket on line 9 blanch pipe after changing gasket he tested pump and the pipe still leaked. Employee was closing a butterfly valve to be able to stop pump and tighten the bolts on the flange a little more. While crouching down to close the butterfly value his hand slipped off the value and he fell forward causing his face to hit the butterfly valve.
