What happened
IE was installing a 33-foot-long 20-gauge deck sheet and the material became stuck at the column splice. IE attempted to free the sheet by prying on the cut notch with a spud wrench to guide it down the flange. The wrench unexpectedly slipped striking IE 's safety glasses and the left side of the forehead.
