What happened
On [REDACTED] at approximately [REDACTED] [REDACTED] leadership was notified of an MHE accident that occurred within the case racks. A [REDACTED] driver was traveling down aisle [REDACTED] when they suddenly came to an abrupt stop causing [REDACTED] to jump the guiderail. The impact caused the employee to strike against the dashboard with their right side and face. At the time of the report the employee stated they felt fine and they were not feeling any pain. However at approximately [REDACTED] the employee notified the Adecco representative that they were feeling pain in their right side and having difficulty breathing. EMS was called shortly after and arrived at [REDACTED]. The employee was transported to a local hospital at [REDACTED] for evaluation. The employee was evaluated at the local ER where they were diagnosed with rib contusions and placed off work for 2 days. During the investigation it was identified that the guiderail had dislodged from the anchorage point. This resulte
