What happened

On [REDACTED] the site was made aware of an incident that occurred on [REDACTED] while an employee was working in Initial Scan. The employee was working in Initial Scan pulling cases from the conveyor onto their workstation when they felt a "pop" in their left thumb. The employee stated they retrieved a case weighing approximately 10 pounds and slid it onto their workstation to keep the line moving when they felt the "pop". The employee sought outside medical treatment and received restrictions. However the proper injury investigation procedures were not followed and the site was unaware of this incident until [REDACTED]. The employee did not work [REDACTED] or [REDACTED] but returned to work on [REDACTED]. The employee had a follow up appointment scheduled for [REDACTED] but they missed their appointment for unknown reasons.
