Before the incident

Team member was moving a 100 pound metal plate from the cleaning tank to the mold assembly bench.

What happened

The employee was lowering a molding plate weighing approximately 100 pounds with a hoist to the mold assembly work bench using the hoist pendant. Immediately prior to setting the plate on the work bench the employee clipped the extension bar of a clamp holding a stack of two plates together weighing approximately 75 pounds each with the plate he was lowering. The plate that was being lowered to the table was landed by the employee on the table and was standing secured in place. The stack of two plates after being bumped was tilted with the extension of the clamp resting on the table and holding the plates up. The employee attempted to secure the plates and stand them straight up from the tilted position. When he reached in to make this risky maneuver the clamp 's extension bar shifted under the weight of the plates and the clamp pulled away from the plates no longer supporting the weight of the two tilted plates. At this moment the two plates fell to the table surface with the employee

The middle and ring finger of the left hand were crushed resulting in partial amputation of the team member 's left ring finger.
