What happened
Employee was walking over to the tool crib for tools for his machine. As he was walking his head was looking down at the paperwork in his hand. There was a forklift approaching the intersection with a coil of wire on the forks. EE was in the blind spot of the forklift operator which caused the forklift operator to strike EE with the wire coil by his foot leg which then made [REDACTED] fall to the ground striking his shoulder against a guardrail. EE suffered a fractured ankle and strain to both shoulders
