The employee was adjusting and centering the punch mechanism during an order change on Machine 1 in the Alpha area.

While making manual adjustments to center the punch on Machine 1 during an order change the employee used an Allen wrench in a vertical position within a tight space. At that moment the punch mechanism began its automatic up-and-down cycle trapping the employee’s right middle finger between the punch blade base and the upper part of the machine causing a crush injury.
