EE was at the final table saw ripping a small block of wood when the fall off piece struck the spinning blade and was kicked back striking the EE in the upper left quadrant abdomen leaving some redness and soreness Grp Supervisor contacted EHS and EE consulted with Medcor Medcor recommended self care and EE insisted on a xray and additional care EE was taken to urgent care where dr [REDACTED] stated Patient will be off work [REDACTED] [REDACTED] and [REDACTED] 14 due to medical reasons Patient fit for duty without restrictions as of [REDACTED] EE has follow up appt on [REDACTED] 25 at BestMed Urgent Care [REDACTED] 15 EE returned with no chief complaint No further action or treatment EE was refered to FFW as part of follow as well Seen on 5 15 [REDACTED] EE elected not to go to the follow up appointment scheduled for [REDACTED]
