Before the incident
Employee (Qualified Mold Changer) was completing mold change on an injection press. Employee raised lower injectors and failed to notice that the lower mold plate was bound to the center plate. After removing the front lower bolts on the guide rails he went to the back of the press to remove the rear lower bolts.
What happened
While loosening the rear lower bolts the lower plate became unbound and fell on employee’s hand.

This resulted in the mold changer getting his fingers crushed causing amputation of left ring and middle fingers.
