Before the incident
On [REDACTED] the Midwest Fleet experienced a Recordable Injury At approximately [REDACTED] a driver (DOH 8 13 18) was transferring empty pallets from their trailer to the pallet trailer in the [REDACTED] [REDACTED] drop lot The driver reported that the plywood to bridge the gap between the two trailers had broken and was of an inadequate size The driver attempted to steady the plywood with their feet As the driver advanced the jack with the stack of pallets towards themselves the pallets made contact with their left foot causing a laceration The driver was transported via ambulance to a local emergency room where they received 7 stitches in their left foot
