Changing over the sqz packs machine

[REDACTED] 1st shift maintenance technician just came to me and said that he was replacing a hydraulic on the squeeze pack machine and while lifting it up he hurt his back. He refused care and said he wanted to go to his chiropractor. I had him sign the employee-self-care acknowledgement form and I sent him home. I informed him to keep in contact with me and let me know how he proceeds.
