Before the incident
Individual was assisting with help positioning a conveyor into its location.
What happened
Once the conveyor was in place the individual was tasked with removing the rigging from the conveyor hook slots. He removed one leg of the chain bridle and began to remove the second. There was a miscommunication between the crew and the rough terrain forklift driver who started lifting the chain causing the hook to catch his fingertip between the hook slot and the hook.
