The injury occurred when an overhanging box on the associate’s cart made contact with a rack near location [REDACTED] causing the equipment to jerk.;;Associate’s Description of the Incident:;The associate stated that while driving down [REDACTED] [REDACTED] a box on their cart made contact with a rack at [REDACTED]. As a result the associate experienced pain rated 5 out of 10 on the pain scale.;;Initial Investigation Findings and Details:;The investigation determined that the injury occurred while the associate was operating equipment down [REDACTED] [REDACTED] and made a sudden stop after one of the boxes on the cart struck a rack. It was found that the cart attached to the associate’s PIV had not been inspected prior to use. Upon notification of the incident the associate accepted a cold pack for immediate care.;;Ite
