Equipment Operation

On [REDACTED] at approximately [REDACTED] a Wayfair associate working in Bulk PUT reported a Near Miss due to being struck by a falling box.;The Witness’s Description of the Incident;The witness states that while working at [REDACTED] and while they were helping another associate restack fallen boxes onto a cart they were grazed (nicked) on the chest by a shifting box before it fell to the ground.;The Initial Investigation Findings and Details;During the investigation the site found that the incident occurred when the associate along with their lead was assisting another associate in restacking boxes that had fallen from their cart. As the associate reached for a box while looking away another box shifted and made contact with their chest before falling to the ground. Both the lead and the reporting associate checked on the associate and they stated they were okay before everyone resumed working.
