Before the incident
Using 12-inch stationary disk metal sander grinder to clean up the edges of a rectangular 1 8-inch-thick piece of sheered metal plate
What happened
Employee was using 12-inch stationary disk metal sander grinder in machine shop to clean up the edges of a rectangular 1 8-inch-thick piece of sheered metal plate (est. 4x7 inches). The employee was holding the metal with the right hand (thumb on top) while guiding rotating with the left hand. The metal was rested against the grinder rest as required. The edge caught on the wheel and pulled into the 1 8-inch gap causing the once perpendicular sheet to rotate upward to the parallel position. The right hand thumb tip was amputated by the metal striking it while flipping up. The metal plate pulled farther into the grinder and exited the grinder on the other side upward into the right side of the employees forehead causing a laceration and a pinhole fracture to the sinus. Employee applied a cloth and pressure to the thumb immediately notified the supervisor and went to the ER with another employee. EHS coordinator was notified and met employees at the ER.
