Before the incident
EE was retrieving a case of stretch wrap from a pallet The pallet was partially empty with all of the cases to the back half of the pallet There was plastic wrap around the base of the pallet that had been cut down as we used the pallet but not removed The pallet was the floor pallet in a 3level rack The team member stepped into the pallet to retrieve a case evidenced by a boot mark on the pallet In doing so they needed to duck beneath the first shelf in the rack As they were stepping back with the case of wrap their foot became entangled in a slot on the pallet or the cutdown plastic wrap at the base of the pallet We are not clear the tripping mechanism When their foot became entangled they lost their balance and as a result straightened up to regain their balance In doing so they struck their head on the on the load and pallet behind them
