Before the incident

Employee was operating Press 9 with his Assistant. They were having troubles with the press throughout the day. The press would go down for unknown reasons and they would have to troubleshoot or the web would break thus causing them to re-web sections of the press causing frustrations. Employee came off break around [REDACTED] and went back to the press. He went to finish what he was needed to do and went to close the overhead door. Out of frustration with the press problems and being denied taking vacation for the rest of the day when he put in his request around [REDACTED] employee attempted to close the overhead oven door three times. On the third attempt he slammed it shut and moved his hands away quickly the door did not latch swinging down and hitting him in the tooth.

What happened

Employee attempted to shut an overhead oven door door was not latching in place out of frustration the employee slammed it on the third attempt and moved his hands quickly out of the way. The over door swing down hitting employee in the tooth.
