What happened
I was welding a plate onto the side of a bucket truck. As I was welding the heat caused the plate to bed outward. I had been using a sledge hammer to pound the plate back onto the truck. On one of my swings my sledge hammer hit the lip of an edge and bounced directly back into my forehead.
