EE was transporting a resident in a wheelchair during a code blue in the west wing hallway heading towards [REDACTED]. employee states that she lost her balance and fell on the floor landing on her right shoulder. Staff reported the incident to HR and i started the preliminary Investigation and check on EE who refused medical treatment and refused to go to urgent care at the time. she advised she will go after work. At this time no sustained injury reported. See incident report attached with witness statement.
