EE Walked into [REDACTED] to get a report from her Co Worker [REDACTED] while walking towards the A bed she felt the floor under her was slippery and started to slide like the floor had ice. she tried to break her fall by grabbing a residents wheelchair but that didn 't stop the fall and she fell to the floor bruising the right side of her of her hip area and bruising her right knee.EE Stated that the floor had some slippery substance in the room but did not know what it was but said it look like a clear oil liquid inside the room.
