Before the incident
The employee was in the process of packing up equipment at the end of the task. While organizing tools and personal equipment the employee placed tools inside a job box. The job box being used had no functioning hinges leaving the lid unsecured and prone to sudden movement.
What happened
While the employee was packing up and placing tools inside the job box the job box lid—due to missing or broken hinges—was not supported in an open position. As the employee’s arm was still positioned near the opening the lid slammed downward unexpectedly.The unsecured lid struck the employee on the right tricep causing immediate pain and minor injury.
