Before the incident
During [REDACTED] start up on D5 operator was in the process of sending flow of [REDACTED] Urethane through the bypass neck of the melt train. Once flow was established through the bypass neck the operators began to detect odors and noticed a haze near the bypass dump neck. Employees notified supervisor and maintenance immediately. EHS and ERT responded to assess the work area. After assessing the area employees who were working in the area were evaluated by ERT. Investigation on-going. (Incident is directly related to INC-264194)
