Before the incident
Helping our maintenance team take bolts out of an extrusion line die face. The bolts were stuck in position and he was using a breaker bar wrench.
What happened
As he was attempting to loosen and take the bolt out it broke free from the die face suddenly causing him to fall backwards. He was on floor level and not elevated but landed on a set of short die stands and the floor. He had some pain in his left side and in his knee after he fell but finished out the day at work.

The incident happened on Friday and by Saturday morning he had pain in his knee and could not put weight on it. injury was sprained left knee.
