1 Prior to the incident the EE was I was hurt before we went on holiday vacation but I paid it no mind When I came back to work it flared up again on [REDACTED] Through the course of me pushing carts I felt something in my shoulder then from that point on the burning sensation was there all again On [REDACTED] I complained and the MERT team offered to call an ambulance or allow me to drive myself I felt I preferred to drive myself I drove there slowly because it was so painful especially in my left shoulder

What happened EE states Im a cart pusher in [REDACTED] and as I was pushing a cart last night I felt a pain in my neck and shoulders It happened when I was trying to turn the cart I feel most of the pain in my Left shoulder and arm3 Where it happened Dept 814 from [REDACTED] to [REDACTED] pushing carts4
